The Patient Information screen displays Patient Information and Patient Alternate Contact Information. Patient information is not editable.

1. You can add or edit Patient Alternate Contact Information:

• Alternate Street Address 1

• Alternate Street Address 2

• Alternate Street Address 3

• Alternate City

• Alternate State

• Alternate Zip

• Alternate Country

• Alternate Home Telephone

• Alternate Work Telephone

• Alternate Mobile Telephone

• Email Address

1. You can select the Preferred Alternate Telephone Number by selecting one of the three radio buttons: Home, Work, or Mobile.
2. You can click the Save button to save the information you entered.
3. You can click the Cancel button and any information you entered is not saved and you are returned to the Patient Lookup screen.
